Grundy County Health Department is looking for participants
to enter the Healthy Lifestyle/Tobacco Free T-Shirt design contest.
The Grundy County Health Departments, Health Education Division would like to
invite all to participate in our Healthy Lifestyle/Tobacco Free T-Shirt design contest.
The program is sponsored through Illinois Department of Public Health, Tobacco
Control and Prevention. “Be creative and inventive with your design reflecting a
Healthy Lifestyle” says Ann Erce, Health Educator. This contest is open to adults
and children
Contest Rules
| 1. All applicants must use one 8.5 x 11 piece of white paper. |
| 2. The design must represent a Healthy Lifestyle/Tobacco Free slogan or theme. |
3. The design may be sent electronically to aerce@grundyhealth.com or mailed to the

| Grundy County Administration Center, |
| Attention: Ann Erce, Health Educator, 1320 Union Street, Morris, IL 60450 |
| 4. The Grundy County Tobacco Control & Prevention Logo must also be included. |
| 5. One design per person, free hand or computer generated. |
| 6. Please include your name, address, phone number, email address and age on the backside of your design. |
The Healthy Lifestyle/Tobacco Free T-Shirt design contest just began; we will be accepting entries until 5p.m. on Monday, October 1, 2007. All entries will be posted
on the Grundy County Health Department bulletin board. The G.C. Health Department Management team will be judging entries on: meeting contest rules, design, creativity and effort taken. There will be awards for first, second and third places. The overall winner design will be printed on T-shirts to be distributed on November 15, during the Great American Smoke Out/Healthy Lifestyle Day at the G.C. Health Department.
